The 1st step:
To start the process, simply download the form, complete the form and send it back to us.

The 2nd. step:
Once you have been notified that the reissueance of your previous I.D. card has been approved, you will receive an email asking you to make a payment via this payment portal.

After payment is received, you will receive a second email asking you to submit a new photograph for use on your new I.D. card along with your preferred mailing address.
-- Upon receipt and approval of your submitted I.D. photograph , your I.D. card will be created and a copy of the proposed I.D. card will be emailed to you for review.
--- After you have approved the I.D. card via your email response, we will send it to the address of your choice.
Unauthorized uses of this PayPal payment portal will NOT be reimbursed.
To set for this exam for a 2nd or at a maximum, a 3rd time, an additional fee will be charged equal to 1/3rd the original fee that is currently being charged. If required, an email with a Paypal payment link will be sent upon request.
